This form is only to be used to change a grade.

If you have a student who would like to receive credit by examination for a course you teach, use this form. This form and a copy of the test is to be turned in to the Office of the Registrar upon completion.

Use this form to document how you verified the identity of your students in fully online courses each semester. Fill out a form for each fully online course you teach. Submit them to your Program Chair and the Director of Institutional Effectiveness near the end of each semester.

This form is used to document instances of plagiarism and academic dishonesty.

Use this form to request color printing privileges. Submit this form to the VP of Instructional Services via email.

Use this blank form when you are asked by Student Services to complete a transcript evaluation on an outside transcript. Once completed, return this form to Student Services. Complete all transcript evaluations in a timely manner. Student will not receive credit for transferred coursework until you complete and submit this form.

Pamlico Community College has a 5 year rule in the General Catalog. It is up to the advisor to accept any courses over 5 years. Normally the only courses not accepted over 5 years are Math and Science courses in technical degree programs. See the college registrar for any questions you may have.

When a student is given an incomplete grade for a course, use this form to document what you and the student have agreed upon as far as completing course requirements. This form is not used to remove an incomplete. There is another form for that process.
